If you change your name, address or telephone number please give full
details, including postcodes, to the reception staff .
REGISTER
WITH PRACTICE?
All new patients over five years old will be offered a consultation with the practice nurse. This allows us to find out about health problems you may have and any repeat prescriptions you require. Your registration with the practice will not be completed until you have attended this consultation. It usually takes three months for your records to come from your former practice.
